Reablement Assistant Purpose of the Role To provide practical, personal and emotional support to individuals in line with their person-centred support plans, enabling them to maintain independence, develop daily living skills and remain safely within their own homes and communities. Key Responsibilities Support service users with agreed tasks and outcomes identified in their individual support plans. Provide personal care, including the safe use of moving and handling equipment in accordance with training and best practice. Assist individuals to develop, maintain and improve daily living skills, using appropriate aids and equipment where required.
