We have an exciting opportunity for an independent, autonomous Prescribing Clinician to play a key role in our Care Coordination and Integrated Urgent Care services across the Bath, Swindon and Wiltshire locality. We are seeking a motivated individual to join our team on a part-time, permanent basis, working regular weekend shifts. You will work across our Care Coordination service to support our services and our Urgent Care service out-of-hours. Our Care Coordination shifts are based at our Chippenham Head Office and our out of hours sessions will be based at one of our Primary Care Centres. This role offers a unique blend of clinical assessment and system navigation, supporting patients to access the right care, in the right place, at the right time, helping to reduce unnecessary hospital admissions and improve patient outcomes.
