Lead a dynamic service that makes a meaningful difference to people transitioning from hospital care. As Team Leader - Step Service, you'll lead a team delivering high-quality accommodation-based support to people with complex health & support needs, helping them move towards safe, sustainable independence. You'll oversee day-to-day service delivery, referrals, risk & safeguarding, while coordinating personalised support & move-on plans. Working closely with Connection Support, hospitals, health teams & other agencies, you'll build strong partnerships & ensure residents receive the right support at the right time.
