You will provide compassionate, person-centred support to individuals with complex needs, creating tailored support and move-on plans while working closely with healthcare, housing and community partners. In this varied role, you'll manage risk, safeguard clients, maintain accurate records, support housing benefit claims and help deliver a safe, welcoming environment.
