Mental Health Community Carer (Driver essential) Job Purpose To provide person-centred support to individuals with mental health needs in their homes and the community, promoting independence, wellbeing, and recovery. Key Responsibilities Provide practical and emotional support to service users. Support individuals with daily living activities, appointments, and community access. Encourage independence, social inclusion, and positive mental wellbeing. Monitor and report any changes in a service user's condition. Maintain accurate records and follow care plans. Work in line with safeguarding, health and safety, and confidentiality policies. Essential Requirements Experience in health and social care or mental health support.
