We are seeking a dedicated professional to provide holistic, person-centred support to a group of key service users linked to local mental health services in Hammersmith and Fulham. Using the recovery approach and SMART support planning, you will help individuals increase independence, maintain accommodation, and enjoy a good quality of life.
